PARTICIPATION AGREEMENT, WAIVER, AND RELEASE OF LIABILITY Organization: Halton Hills Girls Basketball Club Location: Halton Hills, Ontario RISK ACKNOWLEDGMENT I, the undersigned, understand that basketball is a physical sport involving certain inherent risks. These risks include, but are not limited to: Physical contact with other players, equipment, or the floor; Injuries resulting from running, jumping, pivoting, and rapid movements; Serious injury, permanent disability, or death. I voluntarily assume all risks associated with my child's participation in training, games, and events organized by the Halton Hills Girls Basketball Club. RELEASE OF LIABILITY & INDEMNITY In consideration of being allowed to participate, I hereby release, waive, and discharge the Halton Hills Girls Basketball Club, its directors, officers, coaches, volunteers, and the facilities used for training (the "Released Parties") from any and all liability, claims, or causes of action arising out of or related to any loss, damage, personal injury, or death that may be sustained by my child, whether caused by the negligence of the Released Parties or otherwise, while participating in club activities. MEDICAL CONSENT & ROWAN'S LAW • Medical Treatment: I authorize the staff of the Halton Hills Girls Basketball Club to seek medical treatment for my child in the event of an emergency. • Concussion Safety: I acknowledge that I have reviewed the Ontario Ministry of Tourism, Culture and Sport Concussion Awareness Resources (Rowan's Law) and will report any suspected head injuries to the coaching staff immediately. CONSENT TO PHOTO/VIDEO USE I grant permission to the Halton Hills Girls Basketball Club to use photographs or videos of my child for promotional purposes (social media, website, flyers) unless otherwise requested in writing. SIGNATURE & AGREEMENT I have read this document in its entirety and understand that I am giving up substantial legal rights, including the right to sue. I sign it freely and voluntarily.
